Elevate Health merged data from many sources and filled gaps in hundreds of patient records, resulting in 6-million-dollar cost savings.

Elevate Health teamed with Innovaccer to streamline the care delivery process and save expenses with care and population health management solutions. Elevate designed interoperable applications using the Innovaccer Health Cloud's integrated application suite and developer toolkit to build unified patient records for tracking and analyzing patient journeys.

To improve patient outcomes, Elevate created a comprehensive, data-agnostic ecosystem with broad population health stratification tools. It used more than 100 integrations with several digital health services and a vast collection of intelligent APIs with cost, quality, and utilization indicators to acquire insights into its network performance.
